
The Home Medicines Review (HMR) Item 900 is designed to assist patients living at home to maximise the benefits of their medication regimen and prevent medication related problems. There is no age restriction for an HMR.It is not available for in-patients of a hospital, day hospital facility or care recipients in residential aged care facilities.
HMR is a collaborative process between GPs and pharmacists. Once the need for an HMR is established, the GP gives a referral to the patient’s preferred community pharmacy. The community pharmacy arranges for an accredited pharmacist to visit the patient in their home and perform the medication review. The accredited pharmacist reports back to the GP, who then devises a medication management plan with the patient. A copy of the written medication management plan is offered to the patient and a copy provided to the community pharmacist.
