Population Health Program

PNMML's Population Health Program objective is to deliver health promotion messages to primary health care professionalsand the community. With PNMML's long-term goal being to improve health outcomes of the community through general practice, to facilitate greater integration of healthcare, and to educate primary health care professionalsand the community alike on the importance of a population health approach.

Cervical Cancer
PNMML aims to increase the number and proportion of female patients aged 20-69 years attending their General Practitioner for a Pap Smear during the previous 2 year period. We further aim to promote the uptake of the HPV vaccine and thus decrease the number of patients diagnosed with Cervical Cancer.

PNMML promotes numerous resources to GPs including pamphlets and posters as well as the popular Cervix Sampling Card which assists practitioners in identifying visual cervical appearances and includes a guide to taking a high-quality Pap Smear. Health assessment for People Aged 45 to 49 years who are at risk of developing chronic disease (Formerly known as MBS Item 717)
The 45 to 49 (inclusive) year old health assessment is part of the Australian Better Health Initiative (ABHI) announced by the Council of Australian Governments (COAG) in February 2006. ABHI aims to enhance the capacity of the health system to promote good health and reduce the burden of chronic disease.

A health assessment at this stage of life can assist patients to make the necessary lifestyle changes to prevent or delay the onset of chronic disease.

Patient Eligibility and Consent
Eligible patients must be aged 45 to 49 years (inclusive) who are at risk of developing a chronic disease. The decision that a patient is at risk of developing a chronic disease is a clinical judgement made by the GP. However, at least one risk factor must be identified. Factors that the GP may consider include, but are not limited to:

  • Lifestyle risk factors such as smoking, physical inactivity, poor nutrition or alcohol misuse;
  • Biomedical risk factors such as high cholesterol, high blood pressure, impaired glucose metabolism or excess weight; and
  • Family history of a chronic disease.
Components of the Health Assessment - The assessment must include:
  • Information collection, including taking a patient’s history and undertaking relevant examinations and investigations as clinically required;
  • Making an overall assessment of the patient’s health, including the patient’s readiness to make lifestyle changes;
  • Initiating interventions, referrals as clinically indicated; and
  • Providing advice and information to the patient including strategies to achieve lifestyle and behaviour changes (e.g. Lifescripts resources)

Lifescripts resources include lifestyle risk factor assessment and prescription sheets, making them ideal for use with this health check. You can download the Lifescripts resources and the form for ordering hard copies from the Resources Centre of the website.

Services for the 45 to 49 Health Assessment
If further intervention is indicated, consideration can be given to referring to the Metro Healthy Lifestyles Program (MHLP) or an Allied Health Professional.

Coronary Heart Disease
High blood pressure is a major risk factor for a range of cardiovascular diseases, including coronary heart disease. The risk of these diseases increases as the level of blood pressure increases.

PNMML works alongside the Heart Foundation of WA to promote their various campaigns, events and resources to support blood pressure management.

Fresh Start - Quit Smoking Program
Fresh Start is a course for smokers who want to quit but feel they require some support doing so. The course was developed by The Cancer Council Victoria and is a nationally recognised and accredited quit smoking program.

The Fresh Start course comprises 8 one hour sessions over four weeks. Each session covers strategies to assist people in overcoming their smoking addiction. Fresh Start is not just about telling smokers to quit, it looks at why they smoke in the first place and helps them develop strategies to reach their goal of becoming a non-smoker.
PNMML is Accredited by The Australian Council on Healthcare Standards until March 2014.
