Metro Healthy Lifestyles Program (MHLP)

The Metro Healthy Lifestyles Program focuses on supporting people who are newly diagnosed with diabetes (less than 5 years). Clients should be aged 18 years or over and be able to access the program without the need for an interpreter.

The Program is offered to clients who are referred and supported by their GP. Complete the Referral Form – available from the Resources Centre of our website.

Metro Healthy Lifestyles
The Program is offered to patients who are referred and supported by their GP.

Complete the Referral Form - available from the Resources Centre of our website.

For the GP the Metro Healthy Lifestyles Program will offer:

  • Assist clients to take a more preventative approach to their health.
  • Enable them to have further access to locally existing Allied Health Professionals and Community Services (Individual and Group).

The aim of the Program is to maximise the health and wellbeing of clients and thus maintain a healthy lifestyle.

  • Identification of ‘at risk’ patients within General Practice.
  • Initial patient assessment by MHLP Care Coordinator at an arranged time at the Practice.
  • GP Management Plans and Team Care Arrangements.
  • Allied health intervention eg. Individual and/or group sessions.
  • Patient referral to other appropriate locally existing Community Groups/Services.
  • Written progress reports for GPs
  • Assessment, care planning and coordination is FREE.
