Multidisciplinary Diabetes Service (MDS)

Referrals are welcome for people over the age of 18 who have type 1, type 2 for more than 5 years, and Latent Autoimmune Diabetes in Adult (LADA [1.5]).

Please note: patients who are recently diagnosed with Type 2 diabetes will be seen by the team in the Metro Healthy Lifestyle Program.

Multidisciplinary Diabetes Service Education

The program brings together a Diabetes Educator, Dietitian and Pharmacist to support GPs in diabetes care. Clients are provided with a complementary glucometer if self blood glucose monitoring is identified to be suitable. As part of education, sessions utilise the annual cycle of care plan to inform clients of the target guidelines for diabetes. The session provides clients with skills and knowledge on topics such as self blood glucose monitoring and HbA1c, healthy eating, active lifestyle changes, medication and communicating with your Health Care Team.

The Diabetes Educator and Dietitian offer follow up consultations for clients as required, including the scheduled 3 and 12 months review appointments which are undertaken on a one to one appointment or as a group review.

Our experienced team reinforces goal setting to facilitate adopting strategies to empower people with Diabetes in self management skills, enabling clients to make informed lifestyle and behaviour choices and linking in with their local services.

PNMML is Accredited by The Australian Council on Healthcare Standards until March 2014.
