Diabetes Self Management Education
For patients with definite diagnosis of Type 2 Diabetes, newly diagnosed and those requiring ongoing care, not on insulin or incretins, aged 25-70 years of age.
Metro Healthy Lifestyles Program - Program Flyer
Providing practical support for patients with risk factors for Coronary Heart Disease or Type 2 Diabetes to make informed lifestyle and behaviour choices.
Live Well Chronic Disease Self Management Workshops - Program Flyer
OGPN provides a six week self management course for patients with a chronic condition.
Prevention of Type 2 Diabetes
Group education Program which provides information about pre-diabetes, the relationship with lifestyle risk factors, specific strategies for increasing physical activity, losing weight and improving daily nutrition.
Healthy Weight including Healthy Families for Happy Futures (HFHF) - Program Flyer
HFHF is a behaviour change Program for children (6-12 years) with weight concerns and their families. HFHF workshops are facilitated by a dietitian, physiotherapist and a clinical psychologist.
Mental Health
Access To Allied Psychological Services (ATAPS) Program provides minimal cost access to allied psychological services for people experiencing mental health problems.
Youth Health
The Youth Health Program encourages the relationship between young people and GPs and includes activities such as:

- Baby Think It Over – Virtual Parenting Program
- Youth Friendly GP List
Consumer Reference Group
The aim of this group is to develop an ongoing relationship between Osborne GP Network, consumers and community groups in the local area. Representatives meet regularly and provide consumer perspectives on a range of OGPN activities.
