Aged Care Access Initiative The PNMML Aged Care Access Initiative (ACAI) program provides the services of allied health professionals to review residents of RACFs. Speech pathologists and accredited practicing dietitians are available to provide one-one-one clinical sessions within residential aged care facilities. Speech pathology services include assessment and management of communication, and/or swallowing problems. Dietetics services are available for the assessment and management of residents at nutritional risk. An introductory letter outlining the service has been sent to all RACFs in the PNMML area. RACFs can access the services of a dietitian or speech pathologist by completing the required referral form and sending it directly to PNMML. GPs are also encouraged to complete the referral form for residents identified as at risk or requiring assessment. The referral form is available to download from this webpage. A Nutrition Update education session for RACF catering and care staff is also available. This session will discuss key nutrition and hydration issues in RACFs and suggest practical ways to supply extra nourishment to residents who are losing weight and/or are at nutritional risk. |
